As ECG and rhythm data becomes more abundant, accessible, and AI-enabled, electrophysiologists face new questions about trust, actionability, and workflow. The choices of monitoring from single time to fixed uninterrupted, to indefinite interrupted — each with its own technical, clinical, and patient-centered tradeoffs. For example, while trials have shown safety using 48-hour Holter for cessation of post-ablation anticoagulation, their sensitivity is reduced compared to more extended monitoring.
This roundtable will examine what types of monitoring to use, to guide major decisions such as stopping oral anticoagulation, how consumer ECGs and wearables are changing the need for prescribed diagnostic monitors, tradeoffs of positive predictive value based on AI and monitor, and represents an appropriate standard of care given that sensitivity and accuracy will vary based on tool and technology. We will also discuss the impact of clinical decision support tools and opportunities for clinical decision making to occur upstream of EP.
